How to Refer Someone
To make a referral, start by contacting our admissions team by phone or email. We’ll guide you through completing our secure intake form, which helps us match the individual with the right program. Once the form is submitted, our team will review the information and provide confirmation of the next steps within 24 hours.

Proper identification includes Social Security card, birth certificate and documentation of custody (where applicable) of minor children in the family.
